This is a difficult achievement. You have to throw the spoon and while it's flying through the air, it has to spin at least 180° AND you need to catch and grab it in the air again.

1. Enter the (normal) Kidney Surgery and pick up a spoon using two fingers
2. Lower your hand down until you can't lower anymore.
3. Fling you hand up and throw the spoon, then catch it after it falls onto your hand/fingers
4. You've unlocked the achievement "Let's See That Sick Filth Again"
